Affirmed Home Care is looking for a compassionate and dependable Licensed Practical Nurse (LPN) to join our team and provide high-quality one-on-one care to clients throughout New Brunswick and surrounding Central New Jersey areas.

What You’ll Do

• Provide compassionate nursing care based on the client’s individualized care plan
• Administer medications and prescribed treatments
• Monitor the client’s condition and communicate changes appropriately
• Provide wound, catheter, and other skilled nursing care within your scope of practice
• Maintain accurate and timely documentation
• Support the client’s comfort, safety, dignity, and independence
• Communicate professionally with the client, family, and care team

What We’re Looking For

• Active New Jersey LPN License
• 1+ year of nursing experience preferred
• Current CPR/BLS certification
• Strong communication and clinical skills
• Reliable, compassionate, and professional
• Comfortable working independently in a home care setting
• Valid driver’s license and reliable transportation preferred
• EHR/documentation experience preferred
