Location: All Five Boroughs of New York City (Bronx, Brooklyn, Manhattan, Queens, Staten Island) Company: Affirmed Home Care Job Type: Part-Time | Per Diem Compensation: $40.00/hour LPN Wound Care Visits: $80.00 per case Expected Hours: 16–40 hours per week

Responsibilities

• Provide individualized skilled nursing care to pediatric patients in their homes
• Administer medications and treatments as prescribed
• Monitor patient conditions and report significant changes promptly
• Manage specialized medical equipment, including:
• Gastrostomy Tubes (G-Tubes)
• Tracheostomies (Trachs)
• Ventilators
• Collaborate with physicians, caregivers, and interdisciplinary healthcare teams
• Educate families on patient care procedures and safety measures
• Maintain accurate and timely clinical documentation in accordance with agency standards and regulatory requirements

Qualifications

• Current New York State Licensed Practical Nurse (LPN) License – Required
• Minimum of one (1) year of LPN experience – Required
• Pediatric nursing experience – Strongly Preferred
• Fluent in English (verbal and written communication)
• BLS Certification – Preferred
• PALS Certification – Preferred
• Fully vaccinated against COVID-19 – Preferred
• Authorized to work in the United States (ITIN not accepted)
